General Summary

An MSW that provides Social Work services and discharge planning to selected patient populations across the continuum of illness. Promote effective utilization and a seamless transition to post-acute care including nursing home, rehab placements and hospice. Perform crisis intervention with patients and families. Provide Social Work related support to acute Dialysis patients, the homeless and the uninsured.

Essential Functions & Accountabilities

1) Provide Social Work intervention and discharge planning for emergency department patients by facilitating patient connection with a medical home while ensuring appropriate use of ED services.

2) Accurately and completely records all required data in the medical record and departmental data base.

3) Recognizes urgent utilization issues and reports them to the Director of Emergency & Trauma Services.

4) Perform the Medicare Denial process.

5) Participates in the departmental and unit based improvement process on a routine basis

6) Maintains knowledge of community resources available for ED patients and able to provide this information to the ED patients and families

7) Maintains knowledge of community resources concerning referral to Home Care Services including, hospice and arrangement for durable medical equipment.

Candidates must have a Masters in Social Work (MSW). Candidates must have at least one (1) year experience in social work or case work; three (3) year's experience providing social work services in an acute care setting preferred. Must have basic computer literacy. A working knowledge of Microsoft Word, Lotus Notes, clinical database software, and discharge planning preferred
